Practice Information

New Patients

New Patients are welcome to join our practice when they move into the practice area or live in the area. To register with the surgery bring your medical card, if you have one. If you can’t find one, complete a separate registration form for each member of your household, you will need to bring two forms of ID. Complete a new patient questionnaire, included in pack. We invite all new patients to a new patient check with the practice nurse within 6 months of registering. If you have ongoing medical problems please arrange a new patient check within one month.

All patients aged 16 –74 who have not seen a GP in the last three years, and those over 75 in the last year are invited for a health check at the surgery. Please arrange an appointment with the practice nurse.

Location

The surgery is situated at the rear of the bungalow (Whinshiels) on the Main Road in Wrinehill. This is next door to the Hand and Trumpet public house. Access to the surgery is via Cracow Moss. Their is a bus stop outside the Hand and Trumpet.

Practice Area

The practice boundary is in the circle as shown on the Map. Patients that move outside our practice area are asked to register with a new practice. We are unable to keep patients on beyond our boundaries without prejudicing the care of our other patients.

Parking

Patients attending the surgery may use the Hand and Trumpet car park during surgery hours only. Please do not park on the main road, no matter how short your stay; this is a dangerous road with speeding traffic.

Disabled

Disabled patients and those with difficulty in walking may bring their cars to the surgery entrance. Please note there is limited room for parking in front of the surgery, and all other patients should park on the Hand and Trumpet car park. The surgery is fully accessible to the disabled with wheelchairs.

Freedom of Information

The practice complies with the Freedom of Information Act 2000. It gives a general right of access to all types of recorded information held by public authorities. The act sets out exemptions to the right and places certain obligations on public authorities.

Rights and Responsibilities

You have a right to expect a timely, courteous,professional and caring service form all of our team. We aim to treat each individual patient in an equitable fashion, without prejudice, respecting their confidence, dignity and the choices they make. If we fall short of the mark, or you think things could be done better please le us know. Please play your part in helping things run smoothly by taking care of your health, making, keeping and using appointments wisely. Order prescriptions in a timely manner. Give us good notice of any special service you may require and by being patient when we cannot give you an immediate answer to a query or request. We will let patients know if they misuse / abuse our services or staff. After due warning we have the right to remove patients form our practice list. We have a zero tolerance policy for violence and abuse and if needed, to protect patients and staff, we will seek police assistance.

Comments/Complaints

We are always happy for you to tell us when things have gone well. If you have any comments on the services provided you should contact the receptionist or the Doctor. If you have any concerns then we want to know about it and promise to give you a prompt and considered response. There is an information sheet about the process available from reception. Complaints should preferably be in writing and with the consent of the individual patient concerned, wherever possible, and directed to Dr. N. Patel, you may also seek advice from the Patient Advice and Liaison Service (PALS).

Chaperones

Chaperones are offered for sensitive examinations, if you would like a chaperone to be present during an examination please ask.

Your Records Are Safe With Us

As you know, your doctor and the team of health professionals caring for you keep records about your health and the care that you receive. Your records are used in the following ways to guide and administer the care you receive:

  • To ensure that your doctor or nurse has accurate and up to date information to assess your health and decide what care you need when you visit in the future.
  • To ensure that there is full information available should you see another doctor, or be referred to a specialist or another part of the NHS.

The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances:

This requires your consent.

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services e.g. from the social work department.

When we have a duty to others e.g. in child protection cases.

Anonymised patient information will be used at local and national level to help the Primary Care Trust and Government plan services e.g. for diabetic care.  If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff  are bound by the same rules of confidentiality as the medical staff.

If you do not want certain information recorded on your records, please talk to your doctor. All the staff working in the NHS have a legal duty to maintain the highest level of confidentiality about patient information. In some instances, you may be receiving care from other people as well as the NHS. We may need to share some information about you with them, so that we can all work together for your benefit. Unless there are exceptional circumstances, for example when the health or safety of others is at risk, we will not disclose your information to third parties without your permission.

We will only give your relative’s friends and carer’s information if you want us to. If you want to see your records, you should make a written request to the doctor. You are entitled to receive a copy of the information. Please note that a small charge may be made for the administration and time involved.

Should your doctor decide that seeing your records might put your health at risk, you may only be shown part of your records

Medical Students

We now have medical students form Keele University present in the surgery on some days. They may sit in with your GP or practice nurse when you are seen for a consultation, or even see you alone in a consultation under the guidance of a regular GP.

If you would prefer not to have a medical student in your consultation, please advise the receptionist when you are asked for your permission as you arrive for your appointment. Declining to have a student present will in no way affect your treatment.
